How to get on COVID-19 vaccination waitlist

Notice from Howard County Public Health
HOWARD COUNTY - Beginning Feb. 1, the Iowa Department of Public Health has authorized the beginning of COVID-19 Vaccination Phase 1B which includes all Iowans over the age of 65.
However, please note, this does NOT mean that Howard County will receive additional vaccines for Phase 1B by Feb. 1.
At this time, Howard County has NOT received the vaccines to begin vaccinating those in Phase 1B which includes those age 65 and older. As always, when this changes, it will be communicated as soon as possible.
In the meantime, IF YOU ARE 65 OR OLDER and interested in receiving the COVID vaccine, Howard County Public Health has now opened a waitlist for those 65 and older ONLY.
To be placed on this waitlist, go to https://www.surveymonkey.com/r/howardcovaccine and complete the form in its entirety.
If you know someone in Howard County that is 65 and older that may need assistance filling out this form, please assist them if possible. Please note, this waitlist is for those 65 AND OLDER ONLY and being placed on the waitlist does NOT guarantee vaccination. Availability will be based on vaccine allocations received in Howard County from the IDPH. This amount is unknown at this time.
